One page in Adobe Acrobat Reader format required to perform service on Ascent's product line. Completed application is to be fax back to us at 908-523-1808. Upon receiving completed RMA form, Ascent will assign an RMA number and customer will be notified by fax, email, or phone. RMA packages are to be sent to Ascent's Address with the RMA number written on the package along with a copy of Ascent's invoice/ RMA form. Ship RMA packages to:
